Question
I have a patient who was on Prandin
and
Lantus prior
to pregnancy. She is managed on insulin at present time, but will deliver
this month and would like to resume her Prandin post
partum. She is planning to breastfeed. Is Prandin safe to use in lactation?
Answer
Several thoughts:

During breastfeeding, changing caloric intake while the baby's need for
breast milk increases may make it very difficult to control
blood glucose. As such, there should be consideration for the mom to stay
on insulin only until the breast feeding period is over, as the doses can
be readily titrated as needed.

With respect to any medication, and safety for nursing moms, I think it'd
be best to contact the "Medical Information" department
(called by various names by various pharmaceutical companies) and ask them
for updated data. As you probably know, these departments
usually have an 800 number, and may be found in the company's info section
of the PDR.
